misreads what is written or mishears what is said.
Poor handwriting and unclear speech over the telephone are both likely to lead to transcription errors.
Long codes made up of numbers or letters that have no particular meaning to the person keying in the data are particularly vulnerable to error. The skill and accuracy of the typing of the person inputting the data will have a major effect on the number of transcription errors made.
